Abstract
Intramedullary nail fixation is the gold standard for the treatment of lower extremity long bone fractures; however, intramedullary nails are often deformed after inserting into the medullary cavity. With the rapid development of computer graphics, the extraction and registration technology based on 3D point cloud is applied in the preoperative design. This paper presents a method for optimal position localization of intramedullary nail placement into the medullary cavity based on centerline registration. First, the femur and intramedullary nail were scanned using CT and reconstructed three-dimensionally. Second, centerlines of the medullary cavity and the intramedullary nail were extracted respectively. Curvature and best fitting diameter of each control point on the centerline were calculated. Third, the centerline of the medullary cavity isthmus was extracted. The extracted centerline was registered with the centerline of the intramedullary nail in each position. The curvature, diameter and distance measurement of each matching position was calculated. Finally, the best registration position was determined according to a weighted fitness at the individual slide positions. The proposed method was verified by experiments. The experimental results show that the obtained position is the best position for the protection of intramedullary osteogenesis. It is very meaningful to reduce the deformation of intramedullary nails.
